Maple Shade provides services at it's three locations, Mardela Springs, Crisfield, and Princess Anne, Maryland.  Maple Shade provides school based therapy in Somerset and Wicomico Counties.  We provide community and home based appointents in addition to office appointments.   


Mardela Springs Site

Business hours are 9:00am to 5:00pm Monday through Friday. One Saturday a month the Doctor and Therapists are available for appointments.  The office can be reached at (410) 677-0202. All relevant documents can be faxed to (410) 677-0303. We provide individual, family, group, and crisis intervention services. We also have a PRP, MHVP and Respite programs that coordinates with therapeutic services. Dr. Sushma Jani MD and Dr. Niru Jani, MD is available for psychiatric services one Saturday a month.


Crisfield Site382 W Main St  Crisfield, Maryland

Business hours are 8:30am to 5:00pm  Monday through Friday. The office can be reached at (410) 968-3547. All relevant documents can be  faxed to (410) 968-1078. We currently have five therapists serving the three area schools. We provide individual, family, group, and crisis intervention services. We also have a PRP and Respite program that coordinates with therapeutic services. Dr. Sushma Jani MD and Dr. Niru Jani, MD is available for psychiatric services one Saturday a month.

Princess Anne Site -  11793 Somerset Avenue  Princess Anne, Maryland

The office is open by appointment only. The office can be reached at (410) 621-5177. We currently have six therapists serving the five area schools. We provide individual, family, group, and crisis intervention services. We also have a PRP and Respite program that coordinates with therapeutic services. Dr. Sushma Jani MD is available for psychiatric services two Tuesdays a month.

We invite you to make an appointment for your child to have an ADHD assessment using the new Quotient® ADHD Test. If your child has been diagnosed with attention-deficit/hyperactivity disorder, or you think that he or she may be excessively active or has trouble focusing, please consider coming in to see us.

The Quotient ADHD Test gives us objective and accurate data to guide treatment decisions.  The system accurately measures micro-motion andanalyzes shifts in attention state to give a clear picture of the core symptom areas of ADHD: inattention (the difficulty in staying focused and on task), hyperactivity (the inability to control movement and sit still while working), and impulsivity (the inability to inhibit inappropriate responses). The Quotient ADHD Test takes 15 minutes for kids and is administered right in the office. The report is available within minutes of test completion, so we can discuss the results during the same visit.

 

Consider a Quotient ADHD Test if your child seems to have problems with these symptoms:

 

ADHD, predominantly hyperactive-impulsive type

ð     Fidgets with hands or feet; squirms in chair

ð     Has difficulty remaining seated

ð     Runs about or climbs excessively

ð     Difficulty in engaging in activities quietly

ð     Acts as if driven by a motor

ð     Talks excessively

ð     Blurts out answers before questions are completed

ð     Difficulty in waiting or taking turns

ð     Interrupts or intrudes upon others

 

ADHD, combined type

ð     Has both sets of inattention and hyperactive/ impulsive symptoms

 

ADHD, predominantly inattentive type

ð     Fails to give close attention to details or makes careless mistakes

ð     Has difficulty sustaining attention

ð     Does not appear to listen

ð     Struggles to follow through on instructions

ð     Has difficulty with organization

ð     Avoids or dislikes tasks requiring sustained mental effort
