Maple Shade provides services at it's three locations, Mardela Springs, Crisfield, and Princess Anne, Maryland. 
Mardela Springs Site - 23704 Ocean Gateway Mardela Springs, Maryland
Business hours are 9:00am to 5:00pm Monday through Friday. The office can be reached at (410) 677-0202. All relevant documents can be faxed to (410) 677-0303. We provide individual, family, group, and crisis intervention services. We also have a PRP and Respite program that coordinates with therapeutic services. Dr. Sushma Jani MD is available for psychiatric services one Saturday a month.
Crisfield Site - 382 W Main St Crisfield, Maryland
Business hours are 8:30am to 5:00pm Monday through Friday. The office can be reached at (410) 202-2750. All relevant documents can be faxed to (410) 202-2752. We currently have five therapists serving the three area schools. We provide individual, family, group, and crisis intervention services. We also have a PRP and Respite program that coordinates with therapeutic services. Dr. Sushma Jani MD is available for psychiatric services two Saturdays a month.

The office is open by appointment only. The office can be reached at (410) 202-2745. We currently have six therapists serving the five area schools. Dr. Niru Jani MD is available for psychiatric services two Tuesdays a month.
